Physical Therapy

We provide evaluation and treatment of neuromuscular and musculoskeletal conditions to improve movement and function and maximize patient potential for an active and independent lifestyle. Priority is given to active duty personnel, with space available services provided to TRICARE Prime enrollees. All other categories will be eligible for services in accordance with 75 MDG policy. Direct Care Access to active duty military members is available. If you have any muscle or joint pain/injury that is less than 14 days old, Physical Therapy can do an evaluation to aid in your recovery. You do not need a referral from your primary care manager to be evaluated. Physical therapy appointments for active duty personnel are scheduled by the Referral Management Center at 801-586-9571 or 801-777-5414.
