Appointments & Referrals
Video, telephonic and in-person medical appointments for most 75th Medical Group clinics may be scheduled by calling our Central Appointment Line at 801-586-2273 from 7 a.m. to 4 p.m., Monday through Friday. Closed on the second Wednesday every month. Provider teams will look at schedules ahead of time to determine the best option for care (video, phone or in-person) based on the reason and details noted for each appointment. Peak call-in hours are 7 to 8 a.m. Mental Health, ADAPT, and Family Advocacy offices triage and schedule their own appointments (refer to services directory). For triage of COVID symptoms, call the COVID-19 Hotline at 801-777-7934.

For the Nurse Advice Line, call 1-800-TRICARE (874-2273), option 1, or call the clinic at 801-586-CARE (2273) and follow the prompts to the Nurse Advice Line. Active duty patients must receive a verbal authorization for the Nurse Advice Line prior to an urgent care clinic visit.

For any care that your primary care manager does not provide, such as urgent, preventative, mental health, and specialty care, you will need a referral. Find out more about how to obtain the necessary referral and authorization.
