Services Provided

We provide immunizations on a walk-in basis for all eligible beneficiaries during normal duty hours.

Tuberculosis skin tests will not be administered on Thursday or when the 48 to 72 hour reading window falls on a holiday, down day or training day. The last vaccination is given at 4:15 p.m. to allow for a 15-minute observation period after the vaccine is given.

Immunization record requests should be marked Attn: Immunizations and faxed.

COVID-19 Vaccine Availability

The 75th Medical Group is currently offering Initial/Booster COVID vaccines to all Active Duty, GS, Contractors, Dependents and Retirees (Pfizer to Active duty only). Vaccines will be available based upon supply. We are also offering COVID Pfizer vaccine to the 5-11 age group on Thursday and Friday afternoons ONLY.

Vaccine Appointments

An appointment is REQUIRED to receive COVID Vaccinations. All other vaccinations are walk-ins. To schedule your vaccine, please visit: or scan the QR code below with your smart phone. Use the same scheduler to cancel any scheduled appointment.
At times, there have been issues with accessing the site on an AFNET computer.  If you have issues, connect via home computer or smart phone.  With smart phone, it only shows the schedule by day.  If you have issues with the scheduler please call 801-777-5209 or 801-777-7934.

Who qualifies for the vaccine?

All active duty, reservists and guardsmen ON ORDERS, federal employees to include civilian and contract, and TRICARE beneficiaries are eligible. Any family members who are NOT TRICARE beneficiaries can contact their local health department or pharmacy to schedule their vaccine. 

Also, if an appointment is scheduled and needs to be canceled, this can be completed in the scheduler as follows:
  1. Go to the scheduler (
  2. Once on the 75MDG page (, there is a yellow banner at the bottom of the page just above the “Next” button. That banner states: “If you need to cancel a previously made appointment at this facility, please go here.” (See Step 3 for link.)
  3. Click on the “please go here” link:
  4. You will then be asked to enter your first name, last name, DoD ID number and date of birth.
  5. Once completed click “submit request” and your appointment will then be canceled.
Remember: If you or your family member receives a vaccine outside of our clinic, remember to get a record of it and bring to us so we may update your vaccination records.
